Admission Process

Following admission there is a pre-defined twelve week assessment process. Each discipline within our MDT and Clinical team has a pre -determined assessment objective. These objectives encompass: physical, functional, cognitive and emotional elements and are designed to give an overview of the person's current abilities and rehabilitation potential.

Therapeutic interventions to meet individual goals are then formulated and incorporated in to an overall Care plan. Rehabilitation and Therapy intervention commence after admission and form a large part of the continuing assessment process. The assessment pathway acts as a guide in ensuring that all aspects have been thoroughly appraised and evaluated.
