Clinical Services
Clinical Services are organised around individual Care Pathways which are determined on assessment and informed by regular Clinical review. A person-centered Care Plan is developed as part of a fully integrated service model that sees Nursing Care, Rehabilitation and Therapy services provided as a “seamless service”. The profile for Nursing Care: Rehabilitation and Therapy Services is summarised below:-
High Dependency Care
- Multiple, complex and specialist nursing care.
- Tracheotomies, intensive oxygen therapy,
- Partial intermittent PEG or PEJ feeding
- Specialised medical needs.
- Requiring specialist IDT or other equipment
Medium -Long Stay Care
Shelley Park provides a safe, comfortable and supportive residential Care environment for people with a wide range of disabilities and who are unable to live independently.
We also offer:-
- Nursing and therapy services to maintain and promote physical and mental wellbeing.
- Expert Clinical Psychology led services as part of an Emotional support framework.
- Proven success in supporting Neurologically impaired people including individuals who may at times exhibit challenging behavior.
- Established links to Education and Training programmes.
- An active programme of social activities and outings
